The Manor Practice area is bounded by:

Watford Gap Rd, Blake St, Rosemary Hill Rd, Streetly Lane, Skirting Sutton Park to Monmouth Drive, Melrose Avenue, Jockey Rd, Chester Rd, Birmingham Rd, Penns Lane, Walmley Rd, Fox Hollies Rd, Ox Leys Rd, Sutton Coldfield By Pass to Camp Lane.

We can only accept patients who are within our boundary area.

Registration

You can register online using the national service using this link https://gp-registration.nhs.uk/M85033001/gpregistration/landing

Please note that Manor Ashfurlong and Manor James Preston are the same practice at two locations. Whichever surgery you register at, you are able to be seen at both practices. If you have a preference to be seen at a particular site, please let the reception team know when booking.

All new patients are asked to complete a registration form. This gives the Practice invaluable information, particularly when we are waiting for your medical records to reach us from your previous Clinical Commissioning Group.

All patients have a right to express a preference to receive medical services from a particular Doctor or health care professional either generally or in relation to any particular condition. We ask that any such requests are put in writing. We shall endeavour to comply with any reasonable preference expressed, but cannot give an assurance that your preferred practitioner will be available when you need an appointment.

Alternatively you can download the forms to complete at home, and bring them into the surgery.
